First time patients are often surprised at how much time and personal attention they receive from me. I am not the type of doctor who wants to see 110 patients a day for ten minutes each. I want to be able to get to know YOU, not your medical chart. For your first visit, please arrive at least 30 minutes prior to your appointment time to fill out important paper work, such as your medical history. If you would prefer to fill out these documents in the privacy of your own home, please download and complete the following forms prior to your arrival:
- New Patient Packet (Required)
- Alternative Medicine Informed Consent (Required)
- Acupuncture Information Sheet (Optional)
- Acupuncture Informed Consent (Optional)

Please do not wear any perfume or cologne for your visit. Many of my patients are allergic to fragrance and cigarette smoke. I thank you in advance for your consideration of their wellbeing, and look forward to helping you soon!
